PE1 is an enzyme produced specifically by the exocrine pancreas. [Insulin is produced by the endocrine pancreas.] Approximately 90% of the pancreas is made up of exocrine tissue, which produces a broad range of digestive enzymes.

Thus this test is used to detect deficiencies in production of digestive enzymes. It has a sensitivity of 93% and a specificity of 93%. Faecal chymotrypsin evaluation is a similar test, although less accurate as the chymotrypsin degrades rapidly, and results vary due to varying faecal transit times.
